Information we collect
When you request a LEW Consult Pro appointment, the form asks for your name, email address, phone number, desired service, preferred date and time, and general city or ZIP code. Information you voluntarily include in notes may also be received. Please do not submit identification images, financial details, Social Security numbers, medical information, or confidential documents using this request form.
How the information is used
Appointment-request information is used to contact you, determine service eligibility and availability, prepare a quote, and coordinate the requested appointment. Submitting a request does not create a confirmed booking, client relationship, or payment authorization. Only authorized people involved in handling the request should access the information.
Where information is processed
The website uses WordPress and a form service to store and process submissions, and email delivery providers to notify the business. Technical information about site access, such as IP address and browser details, may also be processed by WordPress, security tools, and hosting providers. If a client later chooses remote notarization, that separate process occurs on the designated remote-notarization platform under its own privacy terms.
Your choices and contact
You can request information about, correction of, or deletion of an appointment inquiry by emailing admin@lewconsultpro.com, subject to applicable legal, security, or record-retention requirements. Do not send sensitive identity documents by email unless instructed through an appropriate secure process.
